Health Plan Opt-out

So I opted out of the SFU health plan and now have a negative balance of -$198.00 on SIS.  How do I get the cheque for this money?  I looked at some of the forms on the SFU website but I'm not sure which one to fill out.  Thanks

  • If you want a cheque you can just get it from the registrars office when they process all the refunds. I don't think they will be ready for another week or two though. Wait until your account reads 0, then it should be ready for you to pick it up.
  • Depending on which campus you want to get it from I think you fill out that form. For me, I filled out the Burnaby form, faxed it and they mailed me the cheque after several weeks. But I definitely got it eventually no problem. Burnaby is the only campus that will mail it though. The other campus you must either pick up or direct deposit (which requires setting up with a void cheque or direct deposit form from your bank).

    Speak to the registrar if you need help at any campus or even financial aid advisors can help you if the registrar line is too long (I did that once :P)

    Fill out either of three forms for "For refund requests NOT related to Scholarships and Bursaries".

  • If I choose to have it mailed to me, do they give you any confirmation like an e-mail saying that a cheque was sent or anything like that? Or will the balance just read 0 when they send it?
  • you can always just leave it in your account if you are still planning to take another term at SFU. you can just leave it and $198.00 will be deducted from your next term's tuition. I did that last year!
  • @Andy They will send it and it will probably go to 0 after a few days pass after the mailing of the cheque. Unfortunately there is no confirmation e-mail or anything. Just make sure you fax it right and check back after like 3-4 weeks if you still didn't get it yet.

  • I expect 40% of the amount on the cheque to be direct deposited into my account upon receipt of it for helping you.
